The optimism survey is not the original work of the Carey Group. It is the work of Dr. Martin Seligman (see https://ppc.sas.upenn.edu/). We simply posted an abbreviated version of the survey. The actual, full survey can be found in Martin Seligman’s book Learned Optimism. Any permission or reliability and validity information would need to come from the original source.
